Schedule: Monday-Friday, 8:00 AM-4:30 PM

Join our Value-Based Care team and help patients successfully transition from hospital to home. As a Population Health Care Coordinator I, you'll play a key role in improving patient outcomes, reducing unnecessary readmissions, and ensuring every patient receives thoughtful, connected, high-quality care.
This fully remote role allows you to work from home while still engaging closely with a supportive, collaborative team. We do ask that you live in the Birmingham area and join us occasionally for team meetings and connection.
