Job Overview

The Nursing Facility Service Coordinator will be responsible for identifying, coordinating, and facilitating all necessary support and services for residents of nursing facilities. This role requires excellent communication and interpersonal skills, the ability to manage multiple tasks and priorities, and a strong understanding of nursing facility operations.

Key Responsibilities

1. Manage an active caseload involving monitoring and evaluating options and services to meet individual health needs.
2. Develop and modify care plans at least annually or as needed in collaboration with residents, their families, and facility staff.
3. Ensure residents receive appropriate healthcare services and their medical needs are met.
4. Inform members of their right to choose any willing and qualified provider for services on their plan.
5. Serve as a liaison between residents, families, and healthcare providers.
6. Coordinate and participate in interdisciplinary care conferences as needed.
7. Ensure compliance with all legal and regulatory requirements related to nursing facility services.
8. Complete forty (40) hours of orientation training and an ongoing twenty (20) hour annual training.
9. Follow Amcord Care Inc. policies on document maintenance, confidentiality, and employee rights.
10. Willingness and ability to work in the field and independently in a virtual setting.

Qualifications

• Bachelors degree in social work, psychology, or related fields with practicum experience, or at least three (3) years of experience in a social service or healthcare setting.
• Excellent communication and interpersonal skills.
• Ability to collaborate with staff, participants, and community organizations.
• Strong organizational and problem-solving skills.

*Must be willing and able to travel regularly within the following areas and surrounding counties: Pottsville, Bethlehem, Allentown, Shenandoah, Easton, Reading.*
