The Social Work Case Manager II will support the medical providers, nursing staff, and Neurology clinic patient population in activities related to: social drivers of health, biopsychosocial barriers impacting medical treatment, interoffice consultations with medical providers, patients, and their families on care planning, provide counseling and therapy services with patients as needed Position Summary: Provides clinical therapeutic services; to include therapeutic modalities, brief therapy, and case management services to patients and families. Provides general social work services, as necessary, linking patients and families to appropriate referrals and/or assisting them in meeting their needs.
